Pathway to Purpose Care Services now offer Targeted Case Mangement Services. We are excited to expand the ways that we impact our community. Targeted Case Managers are here to help clients, children/adults advocate for themselves, make informed decisions, provide support, make referrals, and more. These services are covered by Medicaid.
